Effective date: August 8, 2026
These Website Terms and Conditions (“Terms”) govern your access to and use of www.myspiritus.org and related online services operated by Spiritus Dei Wellness Center (“SDWC,” “we,” “us,” or “our”).
By accessing or using this website, you acknowledge that you have read and agree to these Terms. If you do not agree, please discontinue use of the website.
About Spiritus Dei Wellness Center
Spiritus Dei Wellness Center is a nonprofit organization that provides educational programs, faith-informed formation, community initiatives, professional office space, and a directory of independent wellness practitioners and organizations.
SDWC is located at 330 Portland Street, Suite H, Celina, Ohio 45822.
Informational and Educational Purposes
The information presented on this website is provided for general informational, educational, spiritual-formation, and community purposes.
Website content is not intended to constitute medical, mental-health, legal, financial, or other professional advice. Information provided through the website should not be used as a substitute for consultation with an appropriately qualified professional.
Use of this website, attendance at an SDWC program, or communication with SDWC does not create a healthcare provider-patient relationship.
Medical and Emergency Disclaimer
SDWC does not provide healthcare through this website, and its general website forms are not intended for clinical communications.
Do not submit diagnoses, symptoms, treatment information, medical records, insurance information, or other confidential health information through SDWC’s website forms or general email.
This website is not monitored for emergencies. If you are experiencing a medical or safety emergency, call 911. If you are experiencing a mental-health crisis in the United States, call or text 988 or contact an appropriate emergency service.
Independent Practitioners and Organizations
Practitioners and organizations listed in the SDWC directory operate independently. They are responsible for their own:
-
Professional services and clinical decisions.
-
Licensing, credentials, and regulatory compliance.
-
Scheduling and availability.
-
Fees, billing, cancellations, and refunds.
-
Client or patient relationships.
-
Records, confidentiality, and privacy practices.
-
Professional liability and legal obligations.
Unless expressly stated otherwise, a directory listing, office lease, participation in the SDWC professional community, or appearance on this website does not create an employment, agency, partnership, healthcare, or supervisory relationship between SDWC and an independent practitioner.
Visitors are responsible for evaluating whether a particular practitioner or service is appropriate for their needs. SDWC does not guarantee a practitioner’s availability, suitability, services, or outcomes.
Questions about appointments, treatment, professional services, medical records, or practitioner policies must be directed to the individual practitioner or organization.
Programs and Events
SDWC may advertise or provide workshops, presentations, classes, community gatherings, faith-formation opportunities, and other programs or events.
Program descriptions, presenters, dates, locations, availability, fees, and registration requirements may change. SDWC may modify, postpone, relocate, or cancel a program when reasonably necessary.
Additional terms may be presented during registration, including payment, cancellation, refund, participation, conduct, photography, consent, or release provisions. Those event-specific terms will apply to that program. If an event-specific term conflicts with these general Terms, the event-specific term will control for that event.
Participation in some programs may involve ordinary risks associated with travel, group activities, movement, food, facilities, or interaction with other participants. Participants are responsible for reviewing event information and determining whether participation is appropriate for themselves or their children.
Pre-Registration and Notifications
Submitting a pre-registration, interest, waiting-list, or notification form does not complete event registration, reserve a place, guarantee admission, or create an obligation to purchase.
These forms allow SDWC to provide information when registration opens or when additional program details become available. Unless expressly stated otherwise, registration is completed only after the visitor follows the registration process and receives confirmation.
Programs Involving Minors
A parent or legal guardian must submit registrations and provide any required consent for a participant who is a minor.
Programs involving minors may require additional registration forms, emergency-contact information, permissions, conduct expectations, or releases. Those requirements will be presented separately when applicable.
Children under 13 should not submit personal information directly through this website without appropriate parental or guardian involvement.
Payments, Cancellations, and Refunds
If SDWC offers a paid program or event, the applicable price and payment terms will be displayed during registration.
Cancellation and refund policies may vary by program and will be presented with the relevant event information or registration process. Unless otherwise required by law, the event-specific cancellation and refund policy will govern.
Fees charged by independent practitioners or organizations are governed solely by those practitioners’ or organizations’ policies and are not controlled by SDWC.
